Introduction

The immune system protects the body from infections but must also avoid attacking its own cells. This balance is maintained through immune tolerance. While central tolerance removes harmful T cells in the thymus, the discovery of regulatory T cells (Tregs) by Shimon Sakaguchi and the FOXP3 gene by Mary Brunkow and Frederick Ramsdell helped explain how this balance is maintained in the body's periphery.

    • In 1995, Sakaguchi identified a special group of T cells called Regulatory T cells (Tregs) that act like “police,” preventing other immune cells from attacking the body's own tissues.
    • This process is known as Peripheral Immune Tolerance, which ensures the immune system does not overreact against self-cells.

    • In 2001, the FOXP3 gene was found to control the development and function of these Tregs.
    • A mutation in this gene leads to severe autoimmune disorders such as IPEX syndrome in humans, proving its crucial role in maintaining immune balance.

    • These discoveries filled a major gap in understanding how the immune system distinguishes between “self” and “foreign.”

    • Autoimmune diseases: Therapies are now being developed to boost regulatory T cells, which can help calm down overactive immune responses.
    • Organ transplantation: Enhancing Treg activity can prevent rejection of transplanted organs.
    • Cancer treatment: Interestingly, cancer cells exploit regulatory T cells to hide from the immune system. Hence, modern cancer immunotherapies focus on reducing the action of Tregs to make immune attack more effective.
    • This dual role makes Tregs both a friend (in autoimmunity) and a foe (in cancer).
